REGISTRATION INSTRUCTIONS

This will serve as your commitment call.

OPTION 1:REGISTER ONLINE
A. Click on the Excel Version of the Registration Form on the main page.
B. Complete the Registration Form in Excel, and e-mail to hockeycentralflorida@hotmail.com
C. Print and complete an O IM A Cup Waiver and Terms Form for each rostered participant.
D. Bring the completed waivers to the event and submit to the tournament director before first game.
E. Click on the PAY NOW button below, and pay entry fee online.

OPTION 2:REGISTER VIA MAIL
A. Print and Complete (a) 1 Registration Form and (b) 1 Waiver and Terms Form for each participant.
B. Mail the completed Registration Form, all Waivers and the Entry Fee.

C. When the Registration Form is received, you will be e-mailed an entry confirmation.

PAY ENTRY FEE ONLINE
To pay online using Paypal or a Credit Card, please click the appropriate PAY NOW button below.
Send us an e-mail notifying us of your payment from the same e-mail account you entered in PayPal.
There is a small Processing Fee(P.F.) for all Online Entry Fee Payment Transactions.
You may also pay for an individual entry plus a Weekend Warrior (W.W.) membership from USA Hockey Inline.
Be sure to print, complete and bring the completed Weekend Warrior Form to the event with you.
